Individualised Planning and Support Facilitator
An opportunity exists within the Individualised Planning and Support (ISP) program at Case Management Services for an enthusiastic person to work as part of a team that provides individualised planning and support services for people in receipt of Individualised Support Packages. As a dynamic and expanding service, the ISP Program provides increased opportunities and choices for people with a disability by supporting them to remain connected within their communities and networks. As an ISP Facilitator you will need to have experience in person centred planning, possess relevant tertiary qualifications in Social Work, Disability or other relevant discipline. This part-time position will be based on the SACS Award (Social Worker Class 2) including salary packaging as per MCHS policy. Applications should address the selection criteria. Successful applicants will be required to have a National Police Certificate and if relevant to the advertised position a “Working with Children Check” card.
• Capacity to work collaboratively with internal and external stakeholders to achieve organisational goals
• Tertiary qualifications in Social Work, Psychology, Disability Studies, or other appropriate equivalent discipline
• Demonstrated experience in casework practice, including individualized planning with people with a range of disabilities and/or their carers.
• Demonstrated understanding of the State Disability Plan and the Individualised Planning and Support Initiative
• High level of organisational and planning skills including negotiation and problem solving skills, communication and interpersonal skills, literacy, numeracy and IT skills
• Knowledge of Confidentiality and Privacy protocols
• Current Victorian Driver’s Licence
• Tertiary qualifications in Social Work, Psychology, Disability Studies, or other appropriate equivalent discipline
